When You Visit

 

Your First Visit

 

Your first visit to Maine Neurology will, we hope, be a comfortable one. Our staff is always available to answer your questions and discuss our various procedures.

 

Please try to bring to your appointment:

 

  • Your insurance card
  • Physician referral forms (if required by your insurance company)
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent x-rays, test results or appropriate records you may have
  • Any forms you may have downloaded and completed from this web site, or that were sent to you

 

Appointment Lateness and Cancellation

 

  • If you are unable to keep your appointment or are going to be late, please call our office at least 24 hours before your appointment. This courtesy allows us to be of service to other patients who are in need of care.
  • If you are late for your appointment, you may be asked to reschedule for another day.
  • If you are not present for a scheduled appointment, you may be charged a fee. After two no-shows, we may ask you to find another provider outside of Maine Neurology.

 

Prescription Renewal

 

If you need renewals of your current prescriptions, please call our office during normal working hours at least two days in advance. Please do not wait until you are out of medication before calling. Emergency requests for prescription refills after hours and on weekends will be limited to a one-week supply. Controlled medications (tranquilizers, sleeping pills, narcotics) will not be renewed after hours.
